The Vital Records Division is responsible for providing certified birth and death records. Records may only be obtained for persons who were born or died after 1910. Proper identification is required. Birth and death records can be obtained by mail. Please contact Vital Records for details. Birth & Death Records can be obtained at 1145 WEst 5th Ave., Gary, IN 46402. Acceptable forms of identification are a driver's license or picture ID. Fees: Birth Certificate $12, Death Records $10, Amended Affidavit $12, Paternity Upon Marriage Affidavit $15 and Paternity Affidavit $10.
